Forms
Required forms for all teams
- Participant Waiver - Under 18 years of age
- Participant Waiver - 18 years of age or over
- Print Waivers
- Each participant must complete the form entirely
- MUST bring original forms to Orlando and submit at championship check in
- Master Roster (Automatic Download) *Must be completed for each team (division)*
- Click Master Roster to load spreadsheet on to your computer
- Follow example shown on spreadsheet and add your team members (coaches, athletes, ect.)
- Delete ICU example. Must use drop down box to select role
- Save Master Roster spreadsheet and upload into registration form
- International Music Clearance Certificate *Must be completed for each team (division)*
- Download and complete the IMCC Form
Required for Travel Package
- DME (Automatic Download)
- Click "DME" to load spreadsheet on to your computer
- Add your team information for all people staying on ICU Travel Package
- Save DME from and upload into registration form
- Rooming Template (automatic download)
- Click "Rooming Template" to load spreadsheet on to your computer
- Add your team information to the GREEN boxes
- Save you Rooming Template and upload into registration form
